Certificate of Service - Chapter 13 and Amended Chapter 13 Plans

Pursuant to Local Rule 9013-3 every chapter plan must be accompanied by a certificate of service, certifying the manner of service, the date of service, and listing the parties served with the plan. Attaching only the mailing matrix to the plan does not meet the requirements for a certificate of service and the Clerk’s Office may issue a deficiency notice if a mailing matrix is attached without an appropriate certificate of service. Certificate of service language should not be added to Section 8.1 of the plan for Nonstandard Plan Provisions. The certificate of service can be attached to the plan or filed under a separate event.
